Direct Billing / Payment Options

We cannot verify insurance eligibility prior to your scheduled appointment.; It is important that you call your insurance company to find out your benefit information. It is the patient’s responsibility to know how much your out of pocket expense will be, including deductibles, co-pays, allowed number of visits and preauthorization.

The patient acknowledges that they are responsible for full payment of their account. If the insurer issues partial payment or denies the treatment claim, the patient will pay the balance of their account. Payment can be made by cash, debit, credit or by charging to the credit card on file. A receipt will be provided.
